Now more than ever before, the payers have the tools to examine and evaluate patients records. Payers demand to quality documentation has been increasing. This course will guide therapy professionals to align their documentation to match payers demand. The course provides details and feedback from previous practice behavior that lead to major changes to payment system. The course provides detailed learning of the new payment model for home health PDGM. The course outlines a strategy to defend plan of care. Disablement model introduced as conceptual model in the context of clinical practice, the planning and reporting. Inherent complexity and justifying med necessity are the core for quality documentation. The course discusses in detail what is “reasonable” and necessary”, skilled” and “maintenance”. the course guides clinician on the clinical decision making to determine frequency and duration. Clinician will learn how to apply the concept to documentation. Justification of care will require a set of documentation from initial evaluation to discharge. The course provides details about the consequences of improper documentation

1) Learn at least two major upcoming changes in payment to home health care and outpatient
2) Understand the Patient Driven Grouping Model (PDGM) and how it will change home health care payment and practice
3) Learn how past practice and billing behavior still haunting providers
4) Discuss the payor scrutiny to the health care providers, particularly, therapy services
5) Identify healthcare payor demands and align it with patient documentation
6) Describe the components that determine differences between complexity levels
7) Discuss the components of the Patient/Client Management Model
8) Identify four types of goals formulate at least three goals that are measurable and meaningful for their patient’s condition
9) Acquire knowledge on the consequences of improper documentation
